JOB DESCRIPTION

JOB TITLE:                          Social Worker

SECTION:                             Residents Care

REPORTS TO:                     Senior Social Worker

GRADE:                                SWG/PS 2

SUMMARY OF JOB:

Reporting to the Senior Social Worker the incumbent is responsible for administering various forms of interventions and wellness therapy into the lives of the residents of the home that will further enhance the quality of their lives while they reside at the facility.

KEY RESPONSIBILITIES

ADMINISTRATIVE RESPONSIBILITIES

  • Participates in the development of the Annual Work Plan for the Unit.
  • Prepares and submits reports and other documents as requested;

TECHNICAL/PROFESSIONAL RESPONSIBILITIES

  • Update and effectively maintain a data base on all residents;
  • Conduct the orientation of new admissions into the home
  • Conduct skills bank assessment of applicants  upon admission to the Home
  • Provides guidance and effective clinical, gerontological, psychological, mental health counselling to residents with these problems
  • Conducts investigations and prepares case history as a part of residents records for the unit
  • Facilitate the transcribing of reports for residents regarding                               complaints and grievances
  • Report case findings and make recommendations of actions to be taken to the Senior Social Woker;
  • Execute standing procedures regarding the implementation of a programme of rehabilitation of residents into their communities for those who fall into that category;
  • Provides counseling to residents and their family members/next of kin in cases where residents are to be reintegrated into normal family life;
  • Assist the Senior Social Worker in identifying residents financial assets such as pension funds exgratia payment, and re-channel such funds towards their up keep in the Home;
  • Execute occupational and therapeutic activities to enhance the physical, social and psychological well being of the residents;
  • Assist in supervising Undergraduate Social Work Students on practicum placements from the University of the West Indies or any other students on such placements
  • Compile reports regarding Residents being requested for discharge and submit same along with assessments of said cases to the Senior Social Worker, for review and submission to the Discharge Committee

OTHER DUTIES:

Any other duties assigned from time to time by your Supervisor

PERFORMANCE STANDARDS

  • Case findings prepared and submitted within agreed time-frame;
  • Database updated
  • Procedures maintained regarding the rehabilitation and reintegration of residents into communities
  • Relevant counseling and guidance to residents is effectively maintained and targets achieved
  • Investigations conducted as assigned
  • Programmes of recreational occupational and therapeutic activities planned, organized and implemented;
  • Social Work Students adequately supervised;
  • Complaints and Grievance Reports submitted
  • Orientation and assessment of new residents conducted
  • Reports regarding discharge of residents submitted in a timely manner

  • Excellent integrity/ethics excised in the performance of duties.
  • Thorough knowledge of the various areas of social work, including group work, consultation, education and research.  
  • Thorough knowledge of the principles and techniques of social work administration.
  • Good oral and written communication skills

  • Good planning and organizing skills
  • Good knowledge of the Poor Relief Act, Rules and Regulations;
  • Good knowledge of working with the Elderly and or vulnerable groups
  • Proficiency in the relevant computer applications.

MINIMUM REQUIRED QUALIFICATIONS AND EXPERIENCE

  • Bachelor of Science Degree in Social Worker or other related field;
  • Three (3) years experience as a practicing Social Worker
  • One (1) year experience with working with the elderly and or vulnerable groups

Emolument : $4,266,270 per annum
